Please arrive at the check-in time which is 15 mins prior to seeing your provider.
Please bring your insurance cards and photo ID, along with a list of current medications and dosages.
You can spend less time at the doctor’s office by completing any necessary forms at home.

If you are experiencing chest pain, trouble breathing, stroke symptoms, severe abdominal pain, excessive bleeding, severe pain for any reason, or any other potentially life-threatening condition, go to the nearest Emergency Department.
